Your First Appointment

Seeing a new physician for the first time can be unnerving. Our checklist below will help you to remember all the basic items to bring as well as some things you would not think necessary.

Check off each item as you gather it for your appointment… that’s a big load off your mind. You may also print the Appointment Form (PDF).

For your first appointment, please bring:

  • Valid photo ID, such as a driver's license.
  • Completed Patient History form.
  • Your appointment schedule and other materials sent to you.
  • All health insurance and prescription insurance cards.
  • Medical information, copies of pertinent doctors’ notes (be sure to include other medical issues you may have).
  • A list of all doctors with whom you would like us to communicate. Please include the doctor's full name, address and phone number.
  • A list of all prescription and non-prescription medications you take, including vitamins, nutritional supplements, herbal products and over-the-counter drugs.
  • A completed Living Will and Medical Power of Attorney (if applicable but not required).
  • A list of questions you have about your diagnosis, treatment options, or other issues, and pen and paper to write down the answers.
  • Comfortable clothing and shoes as well as a sweater (our air conditioning makes some people feel cold).
  • A local telephone number or cell phone number.
     

It’s also very helpful to bring any previous test results to your first visit.  

These include:

  • Prior reports and films of chest and head/neck X-rays, CAT scans, PET scans, MRI scans or ultrasounds
  • Operative reports
  • Biopsy and pathology reports
  • Results of other recent tests, such as EKGs, blood tests, etc.

Your appointment will be held on the NYU campus in the Bluestone Center for Clinical Research.
